Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County. Sourcewise works in partnership, in contract with Managed Health Care Plans to deliver Enhanced Care Management (ECM) services. ECM is designed to provide care management services to address a wide variety of complex, high-acuity health care-related needs and barriers. ECM Community Outreach Specialist encompasses seven core services : 1. Outreach and Engagement 2. Assists with reporting to health plans 3. Reaching out to clients on RTF list Monthly 4. Comprehensive intake and enrollments 5. Receiving referrals RTF and requesting approval from health plans 6. Coordination of and Referral to Community and Social Support Services 7. Participate in community-based outreach events and presentations
JOB RESPONSIBILITIES
Perform telephonic and in-person outreach for potential participants. Meet with potential participants in various settings. Use research tools and contact providers to locate participants. Build rapport and present ECM program benefits. Explain consent forms and obtain signatures. Utilize required electronic health record system and customer relationship management tools to ensure accurate documentation related to services is captured. Follow confidentiality laws. Participate in outreach events. Ensure accurate data entry and reporting. Assist in audits and compliance. Other duties as assigned.
REQUIREMENTS
Bachelor's Degree or 2+ years equivalent experience. 1-2 years case management experience. Valid California Driver's License. Knowledge of health promotion, care management, and local resources. Strong communication and organizational skills. Ability to work with diverse populations.
LICENSE & CERTIFICATIONS
Valid California Driver's License required. Proof of automobile insurance meeting California State Law minimums required.
