Regional Community Liaison Cottonwood Rehabilitation & Healthcare Center
  • Durango, CO Full-Time | Competitive Pay + Benefits Cottonwood Rehabilitation & Healthcare Center is hiring a Regional Community Liaison based out of Durango, CO .
This position works closely with hospitals, case managers, social workers, referral sources, and community partners while supporting multiple skilled nursing facilities throughout the region. The ideal candidate has experience in healthcare marketing, admissions, community outreach, long-term care, or hospital liaison work and enjoys building relationships in the field.

Responsibilities:
Build and maintain relationships with hospitals and referral sources Work closely with case managers, discharge planners, physicians, and social workers Assist with admissions coordination and patient transitions Educate patients and families on rehabilitation and skilled nursing services Travel between hospitals and facilities throughout the region Collaborate with facility leadership teams regarding admissions and occupancy goals Represent the facilities professionally at community events and networking opportunities Help grow census through strong relationship building and community presence
Qualifications:
Previous experience in healthcare marketing, admissions, hospital liaison, community liaison, or long-term care preferred Skilled nursing or healthcare experience strongly preferred Strong communication and relationship-building skills Self-motivated and comfortable working independently Reliable transportation required Ability to travel throughout the region Apply Today!
