  • Full-time Job Summary Access Community Action Agency is seeking a Benfits Outreach staff to support individuals impacted by economic and benefit changes.
This role focuses on community outreach, intake, and short-term service coordination. The Outreach Navigator connects customers to resources, completes eligibility screenings, and ensures timely referrals to case management for ongoing support. Key Responsibilities Conduct outreach in the community to identify and engage individuals in need of services Provide information on available programs and assist with basic eligibility screening Complete standardized intake forms and collect required demographic data Assist customers with immediate, one-touch services and application support Refer customers with ongoing or complex needs to case management staff Track customer interactions, outcomes, and benefit status in agency systems Schedule and coordinate appointments for follow-up services Support outreach events and track engagement metrics Maintain accurate and timely documentation using Google Workspace and case management systems Follow all confidentiality and compliance requirements Qualifications High school diploma or equivalent required Associate or Bachelor's degree in human services or related field preferred Community Health Worker (CHW) certification preferred Experience in community outreach, customer service, or human services preferred Strong communication and interpersonal skills Ability to work independently and in community-based settings Proficient in Google Workspace (Docs, Sheets, Gmail) Bilingual (English/Spanish) preferred Core Competencies Customer Focus. Work Environment and Physical Demands Combination of office and community-based work Regular travel within Windham and Tolland Counties Ability to sit, stand, walk, and lift up to 25 pounds as needed Ability to use a computer and communicate clearly Reasonable accommodations are available in accordance with the ADA and Connecticut Fair Employment Practices Act.
