Full-Time Salary Range:
$43,000 - $46,000 per year (based on experience and qualifications) Qualifications Minimum High School Diploma required (Associate's or Bachelor's degree preferred) Strong communication and interpersonal skills Ability to work with diverse communities and senior populations Good organizational and documentation skills Basic computer knowledge including Microsoft Office Reliable transportation and willingness to travel for outreach activities and community events Prior experience in community outreach, healthcare, social services, or home care preferred Duties and Responsibilities Responsible for getting involved with community service groups and arranging community gatherings to help educate the public about Sahara Home Care services Assist individuals in obtaining benefits and services, including healthcare and welfare related resources Provide information regarding available benefits and discounts for seniors after assessing their needs and situations Develop plans to improve client wellbeing and evaluate services provided to ensure effectiveness Research and assist clients with applications for state funded programs including SNAP, DHS,TANF, AABD, childcare assistance, and healthcare programs Educate and assist clients in applying for Medicare, Medicaid, housing assistance, and other government programs Advocate for clients and help connect them with resources that improve their quality of life Follow up with clients to ensure their situations have improved and that services are being effectively utilized Maintain professionalism at all times, including communication, responsibility, and appearance Maintain strict confidentiality of client, employee, and company information Participate in company events, outreach activities, and organizational programs Travel to community events, branch locations, and outreach activities as assigned Perform other duties as assigned by management Skills and Abilities Strong community engagement and communication abilities Professional and compassionate approach toward clients and families Strong organizational and multitasking skills Ability to work independently and collaboratively Detail oriented with strong follow through and documentation skills Ability to build relationships with community organizations and agencies Sahara Home Care is an equal opportunity employer committed to a diverse and inclusive workplace.
