- particularly people experiencing homelessness, mental illness or addictions, or struggling with The Street Outreach Coordinator (Community Health Worker
- Outreach & Registration Focus) is responsible for engaging individuals experiencing homelessness and other vulnerable populations through consistent street and site-based outreach.
