ICT Street Team Wichita, KS Position Summary The Outreach Coordinator serves as a frontline representative of ICT Street Team, meeting individuals where they are-on the streets, in encampments, under bridges, in parks, and other community locations. This position is responsible for building trusting relationships with individuals experiencing homelessness while assessing healthcare, social service, and basic needs. Working closely with the Housing Navigator and healthcare team, the Outreach Coordinator conducts new patient intakes, documents identified needs, provides referrals to street clinics and community resources, assists with care coordination, and delivers basic first aid when appropriate. This role requires a compassionate, nonjudgmental approach and the ability to work in challenging outdoor environments throughout the year. Key ResponsibilitiesOutreach and Engagement Conduct street-based outreach in encampments, parks, and other community locations where individuals experiencing homelessness gather. Build rapport and establish trusting relationships with individuals facing complex medical, behavioral health, and social challenges. Engage individuals using trauma-informed, person-centered, and strengths-based practices. Maintain a consistent presence in designated outreach areas and assist with follow-up contacts. Assessment and Intake Conduct new patient intakes and gather demographic, medical, and social needs information. Assess immediate healthcare, housing, hygiene, and safety needs. Accurately document encounters and identified needs. Communicate findings and concerns to the Housing Navigator and healthcare team. Care Coordination and Navigation Connect individuals to ICT Street Team street clinics and other healthcare resources. Assist with referrals to housing programs, shelters, behavioral health services, substance use treatment programs, and other community resources. Support clients in accessing identification documents, transportation, and other services as directed. Coordinate closely with the Housing Navigator to support ongoing client engagement and service planning. Basic Medical Support Provide basic first aid within the scope of training and organizational protocols. Assist with health screenings as directed by licensed healthcare professionals. Recognize urgent medical concerns and facilitate appropriate referrals or emergency response when necessary. Documentation and Reporting Complete timely and accurate documentation of all outreach activities and client interactions. Track referrals, follow-up activities, and outcomes. Participate in team meetings and case discussions as needed. QualificationsRequired High school diploma or equivalent. Experience working with vulnerable populations, including individuals experiencing homelessness, mental illness, substance use disorders, or other barriers to care. Strong interpersonal and communication skills. Ability to work independently and as part of a multidisciplinary team. Valid driver's license, good driving record, and reliable transportation. Ability to maintain confidentiality and professional boundaries. Basic computer and documentation skills. Preferred Experience in outreach, case management, social services, healthcare, emergency medical services, or behavioral health. First Aid and CPR certification (or willingness to obtain). Knowledge of community resources and homeless service systems in Sedgwick County. Experience utilizing trauma-informed care practices. Physical and Environmental Requirements Ability to walk long distances on uneven terrain. Ability to lift and carry outreach supplies weighing up to 30 pounds. Ability to work outdoors in varying weather conditions, including extreme heat, cold, rain, snow, and wind. Ability to safely navigate encampments and other nontraditional work environments. Paid holidays when a holiday falls on a regularly scheduled workday, resulting in approximately 8-12 paid holidays per year depending on work schedule.
