This role offers the opportunity to: Make a direct impact in the lives of individuals receiving services Build strong community partnerships with families, schools, and providers Support person-centered care and self-determination Grow professionally in a collaborative and supportive environment Be part of an organization committed to quality, inclusion, and excellence Position Summary The Community Liaison/HCBS Coordinator plays a critical role in ensuring individuals referred to STEP receive timely, high-quality, person-centered Home and Community Based Services (HCBS). This position coordinates the intake and referral process from referral through assignment while ensuring compliance with Mental Health Code requirements, Medicaid regulations, and best practice standards. The ideal candidate is organized, compassionate, collaborative, and deeply committed to empowering individuals and families through exceptional service coordination. Key Responsibilities Intake & Referral Coordination Coordinate all intake and referral activities for individuals referred to STEP for HCBS services Track and assist with obtaining initial authorizations for services Work closely with the Supports Coordinator Manager to ensure timely assignment and follow-up of cases Collaborate with Supports Coordinators to ensure smooth transitions and continuity of care Community Outreach & Relationship Building Conduct outreach to referring agencies, schools, families, and community partners Represent STEP professionally within the community Promote awareness of STEP's programs and services Advocacy & Quality Support Promote safety, independence, empowerment, self-determination, and well-being for persons served Support compliance with Medicaid Chapter III, MDHHS, DWIHN, Medicaid, and CARF standards Assist with data collection and outcome analysis alongside Clinical and Quality leadership Stay current with industry trends, regulations, legislation, and best practices Professional Engagement Attend required trainings, meetings, and committees Maintain timely and accurate documentation and records Participate in STEP's ongoing growth and development initiatives Qualifications Education Bachelor's Degree in Social Work or a Human Services related field Experience One (1) year of professional experience in human services related to: Mental health services Developmental disabilities Disability supports and services Licensure & Requirements Must meet STEP and DWIHN credentialing/re-credentialing requirements Valid Michigan Driver's License Approval by STEP's Insurance Carrier Reliable personal vehicle for job-related duties Employment Status Non-Union | Hourly Position What Makes You a Great Fit You are: Compassionate and person-centered Organized and detail-oriented Skilled at building relationships and collaborating across teams Passionate about advocacy and community impact Committed to quality care and continuous improvement Join Our Mission At STEP, every interaction matters.
