This is a field-based, sales-driven role responsible for generating referrals, building strong community partnerships, and increasing access to care across the assigned territory. If you're energized by relationship-building, community engagement, and helping individuals navigate complex systems with respect and dignity , this role offers the opportunity to make a meaningful impact while driving measurable growth. This is not a desk-based or primarily virtual role . Success requires consistent time in the field—driving locally, meeting partners face-to-face, and building a strong presence in the community. The ideal candidate brings a builder mindset —someone who is entrepreneurial, driven, and resourceful, and excited by the opportunity to create, grow, and expand a territory rather than step into a fully established one. They are equally comfortable operating as an independent contributor while contributing to a collaborative, mission-driven "one team" culture . This role will serve Circle of Life Home Care, with territory coverage in southern MN / metro Twin Cities; or northern MN.
What you will do:
Territory Growth & Business Development (80%) Own and achieve territory growth goals through referral generation and partner development Execute against both leading (activity) and lagging (referrals, admissions) metrics Proactively prospect and build pipeline through calls, emails, in-person meetings, and presentations Plan and execute a high-volume, field-based outreach schedule , including daily travel and partner visits Build strong, trust-based relationships with case managers, care coordinators, healthcare partners, and community organizations Lead effective meetings by asking open-ended, thoughtful questions to uncover needs and opportunities Clearly and confidently communicate the organization's value proposition to drive referrals Represent One Circle at community events, conferences, provider meetings, and in-service presentations Identify and pursue new opportunities across priority programs and populations Independently manage territory, schedule, and priorities , balancing prospecting, follow-up, and relationship management Operate with an entrepreneurial, driven, and resourceful mindset , adapting to changing needs and market dynamics Build territory presence by creating new opportunities , not just maintaining existing relationships Relationship Management & Market Intelligence (15%) Develop long-term, trust-based partnerships through consistent follow-up and engagement Gather insights from the field to inform strategy and improve responsiveness to community needs Stay informed on services and confidently communicate updates to partners Demonstrate curiosity and initiative by quickly learning new offerings and effectively relaying that information Tracking, Reporting & Cross-Functional Collaboration (5%) Accurately track outreach activity and pipeline progress in CRM (Salesforce) Monitor performance and adjust strategies to meet goals Collaborate with intake, operations, and marketing to support smooth client onboarding and experience Participate in team meetings, trainings, and strategy sessions Align outreach efforts with program capacity and organizational priorities What we are looking for: Bachelor's degree preferred 2-5+ years of experience in sales, business development, outreach, or account management ; or history of demonstrated success in a fast-paced, metrics-driven environment Proven ability to prospect, build pipeline, and convert opportunities into results Training in or exposure to sales best practices (e.g., discovery, pipeline management, consultative selling) strongly preferred Comfortable with both in-person and virtual outreach/selling Experience in healthcare, home care, social services, or Medicaid programs strongly preferred Ability to quickly learn and confidently communicate new concepts and services Experience using CRM systems (preferably Salesforce) Comfortable working in a highly field-based role with regular travel
