This individual will serve as a key liaison between referral sources, healthcare providers, community partners, and our organization to drive admissions, expand market presence, and increase census growth. The ideal candidate possesses strong knowledge of the local healthcare landscape, thrives in a competitive environment, and is passionate about building meaningful relationships that translate into sustainable business growth. This is a results-oriented position for a self-starter who is disciplined, persuasive, energetic, and committed to exceeding expectations. Key Responsibilities Develop and maintain strong relationships with hospitals, physician offices, skilled nursing facilities, rehabilitation centers, assisted living communities, case managers, social workers, discharge planners, and community organizations. Generate qualified referrals and facilitate a seamless transition from referral to admission. Conduct regular in-person visits, presentations, and outreach activities to promote agency services. Identify and pursue new referral opportunities throughout the service area. Educate referral partners regarding available services, eligibility requirements, and admission processes. Track referral activity, conversion rates, and market trends to identify opportunities for growth. Collaborate with clinical, intake, and operations teams to ensure timely admissions and exceptional customer service. Attend networking events, healthcare meetings, and community functions to increase brand awareness and referral generation. Maintain accurate records of referral activities and business development efforts. Meet or exceed established census growth and referral goals. Qualifications Minimum of 2 years of experience in healthcare marketing, admissions, referral coordination, physician relations, or business development. Experience within home health, behavioral health, hospice, healthcare services, or related fields strongly preferred. Demonstrated knowledge of the local healthcare landscape, referral networks, and community resources. Excellent communication, presentation, and relationship-building skills. Strong organizational and time-management abilities. Ability to work independently while maintaining accountability for results. Proficiency with CRM systems, EMRs, Microsoft Office, and data tracking tools. Valid driver's license and reliable transportation.
