Job Description

Under the direction of a supervisory official, the Program Specialist 1 (Outreach Coordinator) designs, plans, coordinates, implements new, and upgrades existing programs regarding opioid response. Specifically, this candidate will be responsible for conducting outreach efforts, onboarding new partners, and maintaining existing partner relationships through continuous engagement as well as conducting initial and follow-up training on Department resource platforms. This candidate will coordinate with a variety of referring organizations within their region to link individuals being discharged from these settings to services in the community through Department-approved platforms. Duties will also include coordinating with receiving treatment organizations to ensure a successful connection and engagement in care; Conduct patient follow-up/check-ins at 24 hours, as well as at 30, 60, and 90 days after the initial contact to confirm that they have successfully connected with the treatment organization; during which candidate will identify barriers preventing individuals from accessing or receiving treatment; and Identify areas for improvement in the overall process. They will act as a subject matter expert and provide solutions to complex issues involving established and new programs expanding awareness and education to the community on available resources during community events and conferences. Position will be hybrid with extensive travel in Northern/Central New Jersey.
