Job Description

Role Summary The Care Manager Outreach Services reports to the Program Director and supports Outreach Services at the program office site, providing general advocacy for low-income families in Sullivan County. This role exists to keep the satellite Outreach Office running smoothly and to connect clients to practical resources. Key responsibilities include overseeing daily office operations; maintaining office procedures with Program Director approval; coordinating training with partner agencies (Health Department, Social Services, Mental Health, Adult Protective Services, Child Welfare, etc.); tracking service statistics for CSBG and preparing monthly Board reports; supporting local volunteer boards with training, planning, and elections; drafting press/radio releases for approval; collaborating with community agencies to develop Outreach programs (Food Stamps outreach, SSI outreach, crisis intervention); participating in fundraising; writing proposals as needed; training and monitoring programs; and serving on relevant community boards.
Qualifications:
Associate Degree in Social Work plus 1 year of experience, or High School Diploma plus 3 years of community services experience Apply if you can manage details, build community connections, and keep accurate records. Typical Workday You start onsite by opening the Outreach Office, checking messages, and setting priorities for the day. You spend time coordinating with local agencies, confirming training details, and preparing materials that need Program Director review. Between client-facing needs and office coordination, you keep notes and update tracking so monthly reporting is ready. Later, you may attend a community meeting or connect with a volunteer board to keep local planning moving. Before leaving, you organize files and confirm next steps for upcoming outreach activities and deadlines.
