DUTIES AND RESPONSIBILITIES
Conducts outreach activities including walkabout, syringe pick-up, mobile van, tabling, and event Conducts community needs assessments to identify new engagement sites in the community Must be able to lift 20 lbs. and carry out community focused street/sidewalk-based outreach Work as an active member of an outreach team of staff, peers, and interns to conduct all required grant services within Alliance waivered outreach areas Acts as an outreach team lead as directed by Assistant Director Provides supportive low-threshold health education, referral, and care coordination services that meet the needs and improves the well-being of participants on-site and in the community Collects accurate data of services and ensures that all data is entered in a timely matter Provides appropriate and expedient overdose response and safety monitoring of participants on-site and in the community Conducts individual and group naloxone and health focused trainings in the community and at Alliance sites Conducts both individual and group services to participant at Alliance sites and in the community Be able to assist in program site service coverage as needed Actively participate in supplies construction, storage, and organization Completes all mandated Alliance and contract specifics trainings Maintains appropriate boundaries with participants, volunteers, and co-workers Conduct duties as required to meet Alliance mission. Attend all required staff meetings, unit meetings, and any strategic planning or trainings required by funded initiative. Attend a minimum of two (2) Agency events for clients and one agency fundraising event every 12 months
QUALIFICATIONS
The Outreach Coordinator will have a High School Diploma and/or at least two years of appropriate experience. Must be willing to work weekends and have a flexible schedule. Must be able to navigate the city by mass transportation and sidewalks with supplies. Spanish or Chinese language speaking skills a plus. Valid NY State drivers license a plus.
Pay:
$45,000.00 per year
