24.88- 27.77/ hour Requirements Required Licenses & Certifications ' Valid driver's license with a driving record that meets organizational guidelines. ' Ability to pass a criminal background check and meet all required screening standards. Education & Experience Requirements ' High school diploma or GED required; Associate's Degree in human services or related field preferred. ' Minimum of 2 years of experience in human services, public sector work, social services, or a related field. ' Experience in community outreach, care coordination, or case management preferred. ' Experience collaborating with community-based organizations, healthcare providers, or social service systems preferred. An equivalent combination of education and experience will be considered. Description Full-time position 40 Hours Per Week ' Hours of work are during normal business hours and may require occasional evenings and weekends. FLSA Status ' Non-exempt Bilingual candidates are eligible for a pay differential. Job Summary The Community Alignment Specialist (CAS) will manage, coordinate and promote the development of community resources in partnership with both public and private agencies within the community. Work includes designing and implementing Family Connects marketing plans for the community. This position is responsible for establishing and maintaining effective coalitions and partnerships among civic, community and health and human services partners to plan programs, services and supports designed to meet the needs of families with newborn children. The position requires independence in planning work, establishing schedules and making program-related decisions. Key Responsibilities ' Build, nurture and maintain strong relationships with community service providers that support families referred by Family Connects nurses. ' Promote the program through outreach efforts including community events, presentations, office visits and social media content. ' Lead or support Community Advisory Board meetings, including agenda development, meeting coordination and documentation. ' Conduct outreach calls to eligible families, explain Family Connects services and schedule participation as appropriate. ' Complete follow-up (Post Visit Connection) calls to evaluate family experiences and resource connections. ' Maintain an up-to-date inventory of community resources, identify service gaps and build partnerships to address those gaps. ' Expand and strengthen referral pathways to increase access to postpartum nurse home visits. ' Monitor incoming referrals and collaborate with the Public Health Nurse Home Visiting Coordinator for triage and assignment. ' Compile and maintain accurate records, reports and documentation to support program evaluation. ' Participate in nurse case conferences, providing consultation and education on available community resources. ' Provide ongoing consultation to nurse home visitors to support effective family resource connections. ' Communicate professionally and collaboratively with team members, clients and community partners, contributing to a positive and team-oriented work environment.Job Classification:
