Description Job Summary:
The Customer Service Rep is responsible for engaging members in preventive health initiatives, facilitating access to care, and building strong relationships with community partners across a designated regional area. This role focuses on three primary responsibilities: conducting outreach and education to promote preventive health services, coordinating care and referrals for members with identified needs, and collaborating with community organizations to improve health outcomes. The position plays a critical role in advancing population health, reducing barriers to care, and supporting the organization's mission to improve overall member wellness.Key Responsibilities:
- Conduct face-to-face outreach to members regarding preventive health, maternity care, care transitions, and chronic condition initiatives
- Educate members on preventive health services and assess needs for care management interventions
- Coordinate access to services by working with members, providers, and community agencies
- Develop and maintain relationships with community centers and organizations to support member engagement
- Assist members in completing Health Risk Assessments and addressing healthcare barriers (e.g., transportation, financial, knowledge gaps)
- Schedule appointments and provide guidance on navigating healthcare services
- Collaborate with human services organizations and state agencies to identify and support at-risk populations
- Provide accurate information regarding organizational benefits and services to members
- Support the development of educational materials for members and providers
- Deliver training on preventive health initiatives, including EPSDT services, to providers and community partners
- Report training outcomes and outreach activities to leadership
- Participate in program development to enhance engagement strategies and reach underserved populations
- Represent the organization at community events and outreach initiatives
Minimum Education & Experience Requirements:
- High School Diploma or GED required; Associate's degree in healthcare, public health, or related field preferred
- Minimum of 2+ years of experience in healthcare outreach, medical assisting, customer service, or a related field
- Prior experience in a medical assistant role required
- Call center or customer-facing experience required
- Experience working with Medicaid or underserved populations preferred
Special Requirements:
- Must obtain required background clearances (e.g., Act 33/34 and 73) prior to start
- Valid driver's license and reliable transportation may be required for community outreach
- Ability to travel locally within the regional service area
Knowledge, Skills, and Abilities:
- Strong knowledge of customer service principles, including needs assessment and service delivery standards
- Ability to communicate effectively, both verbally and in writing, with diverse populations
- Demonstrated ability to work collaboratively in a team environment
- Strong problem-solving skills with the ability to work independently and proactively
- Ability to manage multiple tasks and prioritize effectively in a fast-paced environment
- Cultural competency and sensitivity to diverse populations, including individuals with limited English proficiency
- Understanding of healthcare barriers such as transportation, childcare, and financial limitations
- Basic knowledge of medical terminology
- Proficiency in standard office software and data entry systems
- Demonstrated empathy, patience, and professionalism when interacting with members and stakeholders
Additional Desired Characteristics:
- Bilingual (Spanish-speaking) preferred
- Experience in community outreach, healthcare marketing, or member engagement initiatives
- Familiarity with local community resources and support services
- Experience working with at-risk or underserved populations
Work Environment:
This position operates in a hybrid environment that includes office-based work, community outreach, and field visits within a regional service area. The role may require flexible scheduling, including occasional evenings or weekends, to support community events and member engagement activities. Travel within the region is expected.Other Duties:
