Job Description

Community Outreach Coordinator Monday-Friday 8am-5pm HopeWest is known for going above and beyond for our patients, families, and staff - if you want to be proud to tell others where you work, join our organization! The Community Outreach Coordinator is a community engagement and relationship-management role responsible for increasing awareness, understanding, and appropriate access to HopeWest s full continuum of services through strategic partnerships, education, and community presence. Serving as a key ambassador for HopeWest, this role focuses on stewarding trusted relationships with healthcare partners and community stakeholders to ensure individuals and families are aware of, understand, and can access the right HopeWest services at the right time. The position supports organizational growth by strengthening referral pathways through education, collaboration, and ethical engagement, grounded in HopeWest s mission, philosophy of care, and regulatory requirements. Referral growth is achieved through trust, credibility, and demonstrated community value, not transactional or sales-driven tactics.
MINIMUM QUALIFICATIONS
Education:
Bachelor s degree in healthcare, marketing, business, communications, social work, nursing, or related field preferred.
Experience:
2-3 years of experience in healthcare marketing, sales, business development, admissions, or outreach required. Hospice, home health, or post-acute care marketing experience strongly preferred.
Licenses and Certifications:
RN, LPN, MSW, or other healthcare credentials are preferred.
KNOWLEDGE, SKILLS AND ABILITIES
Builds and sustains professional referral relationships that drive measurable growth aligned with organizational goals. Applies relationship-based sales and marketing strategies effectively within a regulated healthcare environment while upholding ethical standards. Leverages performance metrics, goal setting, and accountability frameworks to guide outreach and business development efforts. Communicates persuasively and compassionately, tailoring messages to diverse clinical, professional, and community audiences. Educates and influences referral partners in complex and emotionally sensitive situations while maintaining professionalism and trust. Analyzes referral data, market trends, and performance outcomes to refine strategies and improve results. Collaborates cross-functionally with clinical, admissions, and marketing teams to ensure alignment and consistent messaging. Represents the organization as a trusted ambassador, demonstrating emotional intelligence, discretion, and professionalism in all interactions.
WORK ENVIRONMENT
This position operates in a hybrid work environment, with the majority of work performed in community-based settings and healthcare facilities, including hospitals, physician offices, skilled nursing facilities, assisted living communities, and other partner locations. Work is performed both indoors and outdoors and requires frequent local travel within the service area, including driving to and between referral partner sites. The role involves regular interaction with clinical professionals, administrators, patients, families, and community stakeholders, often in emotionally sensitive or high-acuity healthcare environments. The work environment is fast-paced and relationship-driven, requiring adaptability, professionalism, and sound judgment while working independently in the field. Periodic work in an office or home-office setting may be required for planning, reporting, virtual meetings, and administrative tasks. Schedule may include occasional early mornings, evenings, or community events based on outreach activities and organizational needs.

Annual Salary:
