Overview:
Join a mission-driven organization supporting a Medicaid-funded social care initiative to improve health equity. This is a non-clinical, contract-to-hire role helping Medicaid members access vital community resources and services.What You'll Do:
- Screen Medicaid members for social needs
- Connect individuals to community & social services
- Perform outreach by phone, virtual, and in-person
- Track referrals and follow-ups in a care coordination system
- Document services and meet productivity goals
What We're Looking For:
- HS diploma/GED required
- 2-3 years in community health, social services, outreach, or care navigation in Suffolk/Nassau County
- Medicaid, SNAP, housing etc. referrals and Community outreach experience in Suffolk County
- Bilingual a plus
- Strong communication & organization skills
- Driver's license & reliable transportation preferred to join a mission-driven team with long-term potential.
