Coordinates and participates in site specific multi-disciplinary planning conferences working with the patient care team to build new patient presentations. Responsible for capturing MPC related data, documentation and linking patients to appropriate community resources (education, transport, financial, etc.) Responsible for scheduling and communication for BJCBH mammography assistance program. Assists the Manager of Clinical Integration and Community Outreach and Director of Oncology Support Services as needed.
