Summary:
This is a temporary FT position ending August 31, 2026. Strategic Partnerships Lead will address the complex challenges of re-engaging people living with HIV who have fallen out of care and early linkage to care through strategic partnerships, capacity development, and training initiatives.
Responsibilities:
Strengthening partnerships with organizations beyond the healthcare setting to access hard-to-reach populations. Work with local organizations to stay abreast and create referral source for clients concerning transportation, shelter, and food distribution. Build collaborations to help identify and address barriers to care, such as stigma, transportation, housing, and other social determinants of health. Develop and maintain a robust network of culturally responsive referral partnerships with shared goals to improve coverage, access, and quality of services for people with HIV. Facilitate training for community health workers and peer navigators from the community to identify and engage those with HIV. Recruit, train, and supervise Peers and Mentors as they support other patients with lived experience. Educate partners on Ryan White
HIV/AIDS
programs in the community. Develop and promote tools, training, and resources for those community organizations serving populations with HIV and those groups at high risk of HIV infection.
Additional Expectations:
Excellent verbal and written communication skills. Ability to remain calm under pressure. Strong leadership skills. Ability to anticipate hazards and problems and respond effectively. Excellent interpersonal skills. Proficient in Microsoft Office Suite or related software.
Requirements:
Qualifications/Experience:
Bachelor's Degree in social services preferred. Two years' experience in community education/outreach.
Time/Physical Requirements:
Monday-Friday, 8:00am-5:00pm/40 hours per week Some evening and weekend activities may require flexing schedule. Tasks occur in both medical office, clinic setting, and community. Able to lift 25lbs and perform normal duties associated in a clinic and office setting.
