Job Description
Overview Keystone Post Acute is seeking a highly motivated, relationship-driven, and clinically knowledgeable Admissions Director to lead admissions, hospital partnerships, referral management, and census development for our Skilled Nursing Facility in Fresno, California. We are looking for a leader who is proactive, organized, clinically aware, and passionate about building strong hospital and community relationships while ensuring appropriate admissions and exceptional customer service. The ideal candidate understands the fast-paced world of post-acute care, can effectively manage referrals from hospitals and case managers, and knows how to balance census growth with clinical appropriateness and operational success. The Admissions Director plays a critical role in both the financial success of the facility and the quality of care we provide. Duties Admissions & Referral Management Manage the entire admissions process from referral through admission Hospital & Community Relations Build and maintain strong relationships with hospital case managers, discharge planners, physicians, and healthcare partners Conduct routine marketing visits and community outreach efforts Represent Keystone professionally within the healthcare community Develop strategies to improve referral conversion rates and increase skilled census Collaborate with leadership on census development initiatives and market growth opportunities Customer Service & Communication Serve as a primary point of contact for residents and families during the admission process Ensure a smooth transition into the facility while maintaining excellent customer service Communicate effectively with interdisciplinary departments to facilitate successful admissions Address concerns quickly and professionally while maintaining a positive facility reputation Operational & Compliance Responsibilities Ensure all admission packets, consents, arbitration forms, PASRRs, and required documentation are completed timely and accurately Maintain compliance with state, federal, HIPAA, and facility regulations Utilize PointClickCare (PCC), referral platforms, and EMR systems effectively Participate in daily stand-up meetings and census reviews Assist with audits related to admissions, authorizations, and payer compliance Review and coordinate incoming referrals from hospitals, HMOs, acute care facilities, and community partners Ensure all admissions are clinically appropriate and financially verified prior to acceptance Coordinate with nursing, therapy, dietary, social services, MDS, and administration regarding admission readiness Verify insurance coverage, authorization requirements, skilled criteria, and payer eligibility Track Medicare, HMO, and overall skilled census goals daily Maintain accurate admission records, agreements, authorizations, and documentation Qualifications Previous experience in Skilled Nursing Facility (SNF) admissions strongly preferred Experience with Medicare, Managed Care/HMO, Medi-Cal, and authorization processes required Strong understanding of post-acute care operations and payer sources Experience with PointClickCare (PCC) or similar EMR systems preferred Excellent organizational, communication, and multitasking skills Ability to work in a fast-paced environment with changing priorities Strong relationship-building and marketing skills Knowledge of hospital discharge planning processes preferred Healthcare, marketing, business, or clinical background preferred Join Keystone Post Acute and become part of a team committed to enriching lives in happy, helpful, and heartfelt ways.
