The Opportunity We are currently seeking a dedicated Nursing Home Hospital Liaison What You Will Do Build and maintain strong relationships with hospital case managers, social workers, discharge planners, and physicians Work closely with our centralized review department to execute a bedside visit with potential admissions candidates (PAC) Present diagnosis specific marketing collaterals to potential nursing home admission candidates and influencers Coordinate facility tours and interaction between PAC/influencer and facility specific staff Collaborate with facility admission teams to ensure efficient and timely admission processes Identify and overcome barriers to admission, including clinical, insurance, and logistical concerns Provide a full time presence in designated hospitals Inform hospital staff, PAC and influencers on Harborview's clinical capabilities and differentiation between levels of care Duties as assigned What You Must Have Active RN/LPN license (or related) in good standing in the state of practice. Experience in a skilled nursing or long-term care setting is preferred but not required—we'll invest in the right heart and mindset.
