Job Description

The Clinical Liaison at Serenity Oaks IRF develops and manages referral relationships with area hospitals, physician offices, and healthcare systems to generate appropriate IRF admissions. This clinically-oriented sales and outreach professional screens patients for IRF candidacy at referring facilities, educates case managers and physicians on Serenity Oaks' rehabilitation program, and facilitates the clinical and administrative admissions process from referral to admission. This role is critical to census development and requires a combination of clinical knowledge, relationship management skills, and an understanding of IRF-PPS admissions criteria. 2.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES
Develop and maintain referral relationships with hospital case managers, social workers, discharge planners, and physicians at target referral sources. Conduct patient screenings at referring facilities to assess IRF candidacy in accordance with CMS 60% Rule and Serenity Oaks admission criteria. Present the Serenity Oaks rehabilitation program to referring providers, educating on admission criteria, services, and outcomes. Coordinate and facilitate smooth clinical handoffs between referring facilities and Serenity Oaks at the time of admission. Communicate clinical screening findings to the Admissions Coordinator and Medical Director for admission decision. Obtain relevant clinical information (H&P, therapy notes, nursing summaries) for admission review. Track referral activity, conversion rates, and market trends; report data to the Admissions Coordinator. Represent Serenity Oaks at community health fairs, hospital events, and provider outreach activities. Coordinate with the Admissions Coordinator on authorization requirements, bed availability, and scheduling. Maintain accurate records of all referral contacts, screenings, and follow-up activities in the CRM or admissions system. Ensure all liaison activities comply with CMS, CARF, and Anti-Kickback Statute requirements. Participate in CARF program evaluation activities related to admissions and marketing. 3.
EDUCATION & EXPERIENCE REQUIREMENTS
Required:
Associate degree in nursing, healthcare, or related field; BSN or clinical bachelor's degree strongly preferred.
Required:
Minimum one (1) year of clinical experience in acute care, rehabilitation, or post-acute settings.
Preferred:
Prior admissions liaison, clinical liaison, or healthcare sales/marketing experience. 4.
LICENSURE, CERTIFICATION & REGISTRATION REQUIREMENTS
Preferred:
Current RN, LPN, RT, PT, OT, COTA, or clinical license.
Required:
Current BLS/CPR certification (AHA or Red Cross) if clinically credentialed. Any held Louisiana clinical license must be verified prior to and throughout employment. 5.
KNOWLEDGE, SKILLS & ABILITIES
Knowledge of IRF admission criteria, 60% Rule, and medical necessity screening. Strong clinical assessment skills for patient candidacy screening at referring facilities. Excellent relationship-building and communication skills with physicians and hospital staff. Understanding of the competitive post-acute market and IRF positioning. Knowledge of Medicare Part A IRF coverage and how to explain it to patients and families. Proficiency with CRM systems, EHR access at referring facilities, and documentation tools. Knowledge of Anti-Kickback Statute and CMS marketing compliance requirements. Self-directed, goal-oriented approach to referral development and census management.
License/Certification:
