Job Description

We are seeking a full-time Admissions Planner to conduct outreach to providers in the community serving veterans and/or their families in order to increase referrals to our Hope for Veterans Program (HVP), and Alfred J. Thomas Home for Veterans (AJT). This position coordinates the full referral and intake process ensuring quick and accessible admission of literally homeless veterans to our programs. Work associated with this position will be conducted in a -including community-based and on-site work. May include evenings and weekends, as needed.

Job Responsibilities:
Identify community service providers, public agencies, veterans' organizations, and veteran service organizations. Conduct meetings and outreach to these and other resources to establish referrals to and increase community awareness of the SSVF Program. Attend County Continuum of Care, Stakeholder, and other relevant meetings in the community to market program, which includes presentations to stakeholders and interested parties. Conducts street outreach when necessary to identify veterans for admission. Participates in marketing activities to increase public awareness our veteran programs. Establishes and maintains cooperative relationships with referral sources. Support veterans in their recovery by coordinating the referral and admissions process and ensuring a smooth and engaging entry into the program. Conducts initial eligibility screening via phone and email. Provides information and referral for mental health, substance abuse, housing, legal, and other social service needs for eligible and ineligible participants. Completes referral forms and pre-admission assessments and collects pertinent referral documents. Coordinates admissions with the Veterans Administration Grant and Per Diem Liaison. Provides orientation to veterans at admission. Includes referral sources, case workers, therapists, family members, or other advocates in the admissions process when authorized by a veteran. Maintains admissions data and required reporting including the Homeless Management Information System (HMIS), census spreadsheets, and utilization reports. Transports veterans as needed in agency vehicles.
Qualifications:
Bachelor's degree in Mental Health or a related discipline as well as 1 year of experience with mental health-related/ substance-use tasks. Knowledge of veterans' issues, mental illness, addiction, and/or homelessness required. Must possess a valid driver's license, with a good driving record. VEVRAA Federal Contractor. Pay:
$29.43 per hour
