What will I do as an Resident Case Navigator with Genacross? Verifies accuracy of resident master information in PCC upon admission and updates as necessary Consults with interdisciplinary team to resolve errors and facilitate proper flow of care Works closely with Ohio Job and Family Services for Medicaid eligible residents and updates system as necessary Assist with Medicaid applications Responsible for obtaining subsequent authorizations and updating progress for managed care. Works closely with Admissions Director during pre-admission process, including insurance verification, medication costs, and obtaining pre-authorizations before the resident arrives Assist Admissions Director with referral management, serving as backup for skilled nursing facility Share weekend on-call duties for referral management, with Admissions Director and Director of Marketing Manage the entire documentation process, including signing admission agreements and ensuring all legal and financial papers are input correctly into the system. Upload admission agreements into PCC Potential for facility tours for prospective residents and their families to highlight differentiators and community amenities Have extensive knowledge of facility policies, residents' rights, and financial responsibilities of residents in facility Attend skilled resident care planning meetings to discuss financial obligations and planning Attend Utilization Review meetings and complete follow up as needed for skilled residents Works closely with Social Service Director to assist in discharge plans related to insurance and financial obligations Reports hazardous incidents, including but not limited to, injuries, equipment, etc. to the appropriate Manager/Supervisor. Implements and follows disaster plans in the event of an emergency. Assumes additional duties and responsibilities as determined by the Director of Nursing.
Resident Case Navigator Requirements :
Minimum of 3 years of experience in a healthcare setting. Minimum of 2 years college education in the field of health care administration, business management, and/or marketing or public relations preferred. Ability to adjust work schedule based on centers' business need, specifically incoming admissions. Familiarity with Microsoft Office products including, but not limited to, Word, Excel, and PowerPoint. Ability to learn new software application as required by the position. Requires excellent organizational skills and good judgement skills. Requires excellent oral and written communication skills, including the ability to interact with senior executives, managers, supervisors and direct care staff, as well as residents, families and physicians.
