  • Seeking top Clinical Liaison to represent skilled nursing facility in Barre, VT
  • Priority Healthcare Group is seeking a dynamic Clinical Liaison to support skilled nursing facilities in the Barre, Vermont markets .
This role is critical in driving referrals, admissions, and census growth by building strong relationships with hospital case managers, social workers, physicians, and other key referral sources. The ideal candidate is a relationship-driven healthcare professional with experience in skilled nursing or post-acute care admissions who thrives in a fast-paced, referral-based environment. Key Responsibilities Drive census growth and maintain occupancy goals. Conduct external marketing to hospital discharge planners, social workers, physicians, and other referral sources to generate admissions. Serve as the primary point of contact for referral partners. Perform clinical reviews of hospital electronic medical records to determine if patient needs can be met. Complete financial screenings, verify benefits, and secure insurance authorizations prior to admission. Coordinate and communicate with facility leadership and admissions teams. Provide tours for prospective patients and families. Participate in community outreach and networking. Identify and pursue market opportunities to improve census and payor mix. Ability to clinically evaluate referrals and ensure patient needs (equipment, medications, care level) can be met. What We're Looking For 2+ years of experience as a Hospital Liaison, Clinical Liaison, or Director of Admissions in skilled nursing or post-acute care . Demonstrated success in increasing admissions and census growth . Strong sales, marketing, and relationship-building skills . Experience working with hospital discharge planners, ALFs, senior centers, and other referral sources . Knowledge of insurance guidelines and financial qualification requirements for SNF admissions . Ability to clinically evaluate referrals and ensure patient needs (equipment, medications, care level) can be met. Why Join Priority Healthcare?
