Duties Conduct thorough resident assessments and diagnostic evaluations to identify social and emotional needs Provide individual, group, and family counseling utilizing evidence-based practices such as behavioral therapy, cognitive behavioral therapy, motivational interviewing, and applied behavior analysis Develop personalized care plans that incorporate mental health counseling, addiction counseling, dementia care, PTSD care, and hospice support Coordinate with multidisciplinary teams to facilitate case management and ensure continuity of care across inpatient, outpatient, home & community settings Manage medical documentation accurately within electronic medical records (EMR) systems while adhering to HIPAA regulations for patient privacy and confidentiality Assist with intake procedures for new patients and facilitate crisis intervention when urgent mental health or behavioral issues arise Support patients with disabilities or developmental challenges by implementing specialized interventions aligned with clinical research standards and best practices Skills Extensive experience in skilled nursing, rehabilitation for geriatric residents and working with individuals with disabilities or developmental disorders such as autism or Alzheimer's disease Proficiency in clinical research methodologies related to mental health and behavioral health fields Strong knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), CPT coding for billing purposes, and familiarity with healthcare regulations including HIPAA Skilled in data collection for clinical documentation, outcome measurement, and quality improvement initiatives Expertise in crisis management techniques for acute behavioral health episodes or trauma-related situations Ability to perform comprehensive patient assessments using diagnostic tools and mental health screening instruments Experience working within EMR systems for accurate record keeping and documentation of patient interactions Excellent communication skills for effective collaboration with healthcare providers, patients, families, and community resources Knowledge of assisted living environments, inpatient facilities, nursing homes, outpatient clinics, special education settings, and post-acute care environments Join us in delivering compassionate social work services that transform lives!
