As the Licensed Social Worker (LSW), you are the bridge between patients, families, and the resources they need. Evaluate psychosocial status of patients and families related to terminal illness and living environmentCare Planning:
Develop and implement interventions based on thorough evaluationsCounseling & Support:
Guide patients and caregivers through stress, coping challenges, and end of life issuesDocumentation:
Maintain accurate, timely clinical records for all referred patientsResource Navigation:
Provide referrals and serve as a liaison with community agenciesTeam Collaboration:
Participate in interdisciplinary care planning and regular team meetingsAdvocacy:
Help physicians and team members understand social and emotional factors impacting careQuality Improvement:
Actively contribute to performance improvement initiativesFlexibility:
Perform additional duties as delegated by the Clinical Supervisor Requirements Bachelor's degree in Social Work from a CSWE-accredited program Minimum 1 year in healthcare, hospice experience preferred Deep understanding of hospice philosophy and end-of-life care Current CPR certification required Valid driver's license, auto insurance, and reliable vehicle for community visits The employer for this position is stated in the job posting.
