Hospice Social Worker

Collect and document data related to patient progress, treatment outcomes, and service utilization for clinical research purposes. Support patients in navigating community resources such as assisted living facilities, home care services, or specialized programs for dementia. Experience Proven experience in hospice care or end-of-life support services with a strong understanding of hospice protocols. Prior inpatient or outpatient clinical experience in hospitals, nursing homes, assisted living facilities, or community health programs is highly desirable. Skills in crisis management, diagnostic evaluation, case management, and working effectively within HIPAA guidelines are essential for success in this role.
