Job Description

Job Overview We are seeking a compassionate and dedicated Hospice Social Worker to join our team and provide essential psychosocial support to patients and their families during one of life's most sensitive times. In this vital role, you will assess emotional, social, and spiritual needs, develop personalized care plans, and coordinate resources to enhance quality of life. Your expertise will help ease the journey for patients facing life-limiting illnesses, ensuring they receive comprehensive, empathetic care that respects their dignity and wishes. This paid position offers an opportunity to make a meaningful difference in the lives of those navigating complex health challenges. Duties Conduct thorough patient assessments to identify emotional, social, and behavioral needs related to hospice care. Develop and implement individualized care plans that incorporate mental health counseling, crisis intervention, and family support strategies. Collaborate with interdisciplinary teams—including medical staff, nurses, and therapists—to coordinate holistic patient care. Collect and document data related to patient progress, treatment outcomes, and service utilization for clinical research purposes. Support patients in navigating community resources such as assisted living facilities, home care services, or specialized programs for dementia. Experience Proven experience in hospice care or end-of-life support services with a strong understanding of hospice protocols. Prior inpatient or outpatient clinical experience in hospitals, nursing homes, assisted living facilities, or community health programs is highly desirable. Skills in crisis management, diagnostic evaluation, case management, and working effectively within HIPAA guidelines are essential for success in this role.
