Job Description
•Medical/Clinical Social Work Experience Needed•Ironwood Cancer & Research Centers has beautiful state-of-the-art integrated Cancer and Breast Centers, with locations strategically located throughout the Valley. Each one provides a superior cancer care environment with a full spectrum of cancer related services for patients. Our multi-disciplinary team approach includes surgical oncology, medical oncology, radiation oncology, breast centers, diagnostic imaging services, social service support, nutritionist, integrative services, and genetic counseling.

Provide social services to our patients and families at our West Valley offices (Glendale, Goodyear, Surprise) as they navigate their cancer diagnosis and treatment journey.Essential Duties and Responsibilities:
Works closely with medical team to help patients understand their diagnosis and treatment plan. Active participant in the supportive care services team. Electronic documentation in patient's medical record. Facilitate support groups and educational in-services. Assist patients with community resources for health-related social needs such as living situation, food, transportation, utilities, and safety. Contribute to patient education activities including lunch and learn presentations and newsletter blogs. Support operations of boutiques for wigs and pantry resources. Support volunteer program. Assist patients with financial applications for AHCCCS, disability, and charitable foundation grants. Assist patients with legal documents like advanced directives, living wills, and medical powers of attorney. Flexibility with daily schedule to accommodate some weekend and evening classes. Must be flexible and willing to travel to different offices based on the needs of the patients. Minimum Qualifications Able to function independently and as a member of the supportive care services team. Knowledge of and experience with community resources Excellent knowledge of community resources and referrals, .i.e., Palliative care, Medicare, AHCCCS, ALTCS, home health, rehabilitation, hospice etc. Knowledge of medical continuum of care: home health, hospice care, nursing, and rehabilitation Comfortable independently coordinating and facilitating support groups and classes for oncology patients. Previous medical social work and oncology experience preferred. Culturally and ethically sensitive to patients' diverse needs and planning. Education and/orExperience:
Bachelor's Degree or Master's Degree in social work with state licensure. Oncology and medical experience preferred.Certificates, Licenses, Registrations:
