The Direct Support Team Coordinator is the perfect position for a determined, social worker or behavioral health professional looking for an exciting and rewarding career opportunity supervising teams supporting young people to overcome challenges and recognize their potential . This role is a unique blend of program leadership and clinical work, allowing for those who prefer to remain anchored in working with families to also expand their leadership abilities. The emphasis of this role is to serve a small caseload in order to prioritize intensive work delivered through creative designs established by the coordinator in collaboration with their teammates, children and the families they serve. Direct Support Team Coordinator Job Responsibilities:
This position is responsible for providing assessment and creating unique interventions to support young people who deserve an opportunity to experience a sense of normalcy and community life. The applicant must demonstrate an ability to relate to and build positive connections with children and adults, be comfortable working in a strength-based and family-centered way and be able to work effectively within a team environment. Coordinators must demonstrate an ability to effectively communicate with families, Child and Family Teams members (stakeholders, therapists, case managers, etc.) and agency management. Familiarity with Child and Family Teams and ability to provide clinical supervision is also required. In addition candidates need experience working with children and families as a Care Coordinator, Intensive Case Manager, Clinical Liaison, Supervisor, or Team Lead with experience in home and community-based support services. Other key functions of this position include: facilitating team meetings, performing family intakes and writing assessments/support plans, measuring progress and outcomes of services provided, promoting employee development and evaluating performance. Flexibility in scheduling, willingness to drive to different cities, and work within the homes/communities of the families we work with is required. Requires afternoon, evening, and weekend work. This position will also participate in an on-call rotation. Performing other duties related to the position This position requires afternoon, evening, and weekend work. Adaptability in scheduling, driving throughout the county and working within the homes and communities of the families we support is required.Education/Experience Needed:
High school diploma or GED Minimum of five (5) years related work experience or Related Bachelor's degree Supervisory experience preferred. Bilingual (Spanish) requiredRequirements:
Must be 21 years or older Valid Arizona Driver's License with an acceptable 39 month driving record Reliable transportation, including proof of Insurance Candidates are required to submit a negative drug test Obtain an Arizona DPS Level One Fingerprint Clearance Card Complete Adult, Child, and Infant CPR and First Aid trainingCompensation:
