The ideal candidate will have experience in case management, assisted living, research, special education, and group therapy. Experience in hospice, behavioral health and familiarity with medical records is highly desirable. ## Duties
  • Conduct assessments to identify clients' needs and develop individualized care plans
  • Provide counseling and support to individuals and families facing various challenges
  • Collaborate with other professionals to coordinate services for clients
  • Advocate for clients to ensure they receive necessary resources and support
  • Maintain accurate and up-to-date client records and documentation
  • Participate in multidisciplinary team meetings to discuss client progress and treatment plans ## Skills
  • Strong communication and interpersonal skills
  • Empathy and understanding towards diverse populations
  • Ability to work effectively with individuals of all ages, including children
  • Proficiency in case management software and electronic health records systems
