Job Summary and Responsibilities As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.

Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment. Provides clinical assessment, reassessment, and psychosocial support as appropriate to diverse populations. Provides psychotherapy for the purpose of assessment, diagnosis, and treatment of patients. Provides crisis intervention, conflict resolution, and psychoeducation to patients and families. Provides consultation, psychoeducation, and psychosocial support to other health care professionals. Assess for abuse/neglect in vulnerable populations and makes mandatory reporting as required. Provides community resource coordination and referrals for patients as needed. Job Requirements Required Masters Degree in Social Work One (1) year medical or hospital social work experience, including post Masters and/or supervised internship Experience in psychosocial assessments and safety planning, with a wide variety of populations Proficiency in completing Arizona Revised Statute Title 36 legal documentation and the burden of evidence required to support each of the subtypes of petition Efficient documentation and communication skills Knowledge of Social Work Code of Ethics, National Association of Social Work (NASW) Standards of Practice, Arizona Revised Statute Title 36, Arizona Regional Behavioral Health Authorities (RBHA) system with specific expertise in Maricopa County RBHA providers and Seriously Mentally Ill (SMI) services Familiarity with the psychiatric system of care on a local, regional, and national level to allow for effective transitions of care
Master Social Worker:
AZ Preferred Two (2) years post-masters experience: medical, hospital social work, or out- patient primary care setting experience Allscripts and Cerner experience Knowledge of best practice in clinical social work and regulationstandards within a healthcare setting
