Responsibilities:
Perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families. Serve on hospital committees and develop and present in-services to Hospital staff. Dignity Health now offers an Education Benefit program for benefit-eligible employees after 180 days. This program provides debt relief and student loan assistance to help you achieve your goals. Full-time employees can receive up to $18,000 over five years, while part-time employees can receive up to $9,000. 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting LMSW license from the AZ Board of Behavioral Health Examiners Master's Degree from a School of Social work accredited by the Council of Social Work Education Ability to work independently as needed with a high degree of detail orientation Ability to work efficiently in a fast-paced environment with changing priorities Ability to work collaboratively with an interdisciplinary care teamPreferred:
