Social Worker, MSW (Spanish) Golden Years Adult Day Health Care Job Category Social Services Overview Description Golden Years ADHC is looking for a Bilingual Social Worker/Social Worker Assistant who is responsible for daily interventions and care planning of clients with multiple medical and psycho-social problems. The Medical Social Worker/Social Worker Assistant plans for and monitors services, interventions ensuring provision of quality care, plays role of a liaison between participant, family, caregivers and professionals.

MINIMUM QUALIFICATIONS

Master's degree or Bachelor's degree from accredited school in Social work, Psychology or human related services Bilingual English / Mandarin or Spanish Ability to work with individuals with physical or mental limitations

ESSENTIAL FUNCTIONS/RESPONSIBILITIES

Timely documentation of assessments, reassessments, quarterlies, flow sheets for each participant. Researches and coordinates community referrals. Monitors and reports case recording on a timely basis. Participates/leads in coordination of social/health services. Participates in Multi Disciplinary team meetings and coordinate care of each participant.
