Job Description

SUMMARY The Social Worker provides case management, treatment coordination, crisis intervention, and supportive services to youth and non-minor dependents residing in Brighter Horizon Treatment Center's Short-Term Residential Therapeutic Program (STRTP). Working collaboratively with the Clinical Supervisor, Head of Services, county representatives, and multidisciplinary treatment team, the Social Worker ensures that each client receives services consistent with their treatment plan, placement goals, county requirements, and applicable Title 22 and STRTP regulations. The Social Worker serves as a primary liaison between the client, family, county agencies, schools, medical providers, and other community resources to support successful treatment outcomes and permanency planning.
TECHNICAL SKILLS
Knowledge of trauma-informed care principles. Knowledge of child welfare, mental health, and STRTP regulations. Strong assessment, problem-solving, and organizational skills. Excellent verbal and written communication skills. Ability to work collaboratively within a multidisciplinary team. Ability to maintain professional boundaries and confidentiality. Proficiency with electronic health records and Microsoft Office applications.
CERTIFICATIONS
Licensed Clinical Social Worker (LCSW) Licensed Marriage and Family Therapist (LMFT) Licensed Professional Clinical Counselor (LPCC) Valid California Driver's License. Reliable transportation and proof of automobile insurance. Successful completion of DOJ, FBI, and Child Abuse Central Index (CACI) clearances. Successful completion of pre-employment health screening and TB clearance. Current CPR/First Aid certification or ability to obtain certification upon hire. Completion of all agency-required trainings.
ESSENTIAL DUTIES AND RESPONSIBILITIES
  • CASE
MANAGEMENT AND CLIENT SERVICES
  • Provide ongoing case management services for assigned clients.
  • Conduct regular face-to-face contacts with clients and document all interactions.
  • Monitor client progress toward treatment goals and objectives.
  • Assist clients in developing independent living, social, emotional, and behavioral skills.
  • Support clients in accessing educational, medical, mental health, vocational, and community resources.
  • Participate in treatment planning and multidisciplinary team meetings.
  • Provide crisis intervention and support during behavioral, emotional, or placement-related emergencies.
  • Assist with discharge planning and transition services.

  • Complete and maintain all required client documentation within established timelines.
  • Develop, update, and monitor treatment plans in collaboration with the treatment team.
  • Prepare and submit:
  • Intake assessments

  • 30-Day Reports
  • Quarterly Reports
  • Social Worker contact notes
  • Progress notes
  • Case management documentation
  • Discharge summaries
  • Other required county and agency documentation
  • Ensure documentation meets agency, county, Medi-Cal, and regulatory standards.

  • Maintain regular communication with:

  • Family Members and Support Systems
  • Participate in county staffing meetings, Child and Family Team meetings, and court-related activities as required.
  • Coordinate services to ensure continuity of care and compliance with placement requirements.
ESSENTIAL DUTIES AND RESPONSIBILITIES
  • PROGRAM SUPPORT
  • Assist with client admissions and intake processes.
  • Support facility staff in understanding treatment goals and client needs.
  • Participate in quality assurance, program improvement, and compliance activities.
  • Maintain confidentiality in accordance with HIPAA, state regulations, and agency policies.
  • Perform other duties as assigned.
REQUIRED QUALIFICATIONS
  • EDUCATION Master's degree from an accredited college or university in one of the following fields:
  • Social Work
  • Counseling Psychology
  • Clinical Psychology
  • Marriage and Family Therapy
  • Social Welfare
  • Human Services
  • Human Development
  • Criminal Justice
  • Education with an emphasis in Counseling
  • Related behavioral health field
REQUIRED QUALIFICATIONS
  • EXPERIENCE
  • Minimum one (1) year of professional experience providing social work, case management, behavioral health, child welfare, juvenile justice, or related services.
  • Experience working with youth, foster youth, non-minor dependents, probation-involved youth, or individuals with behavioral health needs.
  • Experience developing and maintaining clinical documentation.
PREFERRED QUALIFICATIONS
  • Experience in residential treatment, STRTP, foster care, or behavioral health programs.
  • Experience with Medi-Cal documentation and billing requirements.
  • Bilingual skills preferred.
PHYSICAL REQUIREMENTS
  • Ability to sit, stand, walk, bend, and reach for extended periods.
  • Ability to transport and lift materials weighing up to 20 pounds.
  • Ability to participate in recreational and community-based activities with clients.
  • Ability to respond appropriately during emergencies and crisis situations.
WORK ENVIRONMENT
The work environment includes residential treatment facilities, community settings, schools, county offices, and other service locations. Employees may occasionally work outdoors and may be exposed to moderate noise levels. Evening, weekend, and on-call responsibilities may be required.
