Social Work Utilization Review Intake Job Description:
The Psychiatric Case Manager coordinates the care of patients through the provision of clinical case management to assure that patients are being treated at the appropriate level of care and that the admission, treatment and discharge are well coordinated. This is accomplished through close collaboration with the patient, family, physician, treatment team, external case managers, referring therapists, outpatient clinic, and payor.
Preferred experience includes:
experience working with severely mentally ill patients, competency in completing psychosocial assessments, advanced skills and knowledge in discharge and safety planning, working knowledge of current reimbursement requirements (including Medicare, MediCal, private contracting), excellent communication skills, both written and verbal, ability to work with a broad spectrum of health care professionals, assertive and creative approach to solving complex systems problems, and ability to work under pressure and deal with change.
