JOB DETAILS POSITION SUMMARY
•LCSW Social Worker
POSITION DUTIES
•Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.
MINIMUM REQUIRED QUALIFICATIONS
•CA
LCSW BLS 5150
certified in CA 2 years recent acute hospital experience in specialty Must have prior Dignity experience to be considered
LENGTH OF ASSIGNMENT
•13 Weeks
SHIFT / HOURS PER WEEK
•Varied 8hr shifts, EOW SYSTEMS•Cerner
