COMMUNITY ACCESS NETWORK
is a non-profit organization providing Mental Health Counseling, Therapeutic Behavioral Services, Short Term Residential Therapeutic Programs, and a Foster Family Agency. The agency is seeking qualified candidates for the FFA Social Worker position.
Summary of Job Duties:
Social Work personnel shall be employed to provide the social services of the foster family agency.
Job Requirements:
Develop needs assessment and treatment plans for each child on their caseload, and update on a quarterly basis. Meet with each child weekly, or more frequently as specified in their treatment plan. Ensure that certified foster parents have the support needed to make the child's placement successful. Facilitate reunification plans/efforts whenever possible, including facilitating monitored visits as needed. Document and provide pertinent information to the placement workers of all children monthly. Participate in the quarterly foster parent in-service training. Evaluate and assess child's progress and document progress on the Quarterly Report in a timely manner (no later than 5 days after the expiration of the quarter.) Supervise the certified resource parent(s) monthly to ensure that all Title 22 regulations and the department of social services policies are adhered to. Always maintain up-to -date children's files. Monitor child's grades and academic functioning. Weekly in-office duty day and 24-hour on-call-services on a rotational basis. Retrieve all required documents from the certified home monthly. Participate in all scheduled in-service training. Additional duties as assigned by the FFA Coordinator. Conduct self-audit of charts and return to FFA Coordinator and Quality Assurance Specialist.
Specialized Skills:
Sensitivity in working with a vulnerable population Skilled in communicating effectively, both orally and written, with a variety of people of various socio-economic and educational backgrounds. Ability to correctly interpret and apply rules, regulations, policies, and procedures in accordance with Title 22. Displays a high level of ethical conduct and exhibits honesty and integrity. Displays a high level of initiative, effort, and commitment towards completing assignments efficiently. Demonstrates responsible behavior and attention to detail. Responds appropriately to supervision and follows the established policy and procedures Knowledge of the principles, practices, techniques, and professional standards in the field of social work.
