GENERAL PURPOSE
Under the administrative supervision of the Team Lead/Program Director, this position is responsible for the treatment plan and development based on the strengths and expressed needs of the child and his or her parents.
DISTINGUISHING CHARACTERISTICS
This is an at-will direct service position within a program. The position is responsible for assisting and advocating for our members in all areas of treatment and help them apply for and receive services.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Works in partnership with the provider and the parents who are treated as experts concerning their families. Completes intake paperwork; conducts behavioral and strength assessment. Facilitates meetings with the child, family, lead provider and other stakeholders. Collaborates on treatment plans with the lead provider. Responsible for assisting family and lead provider in implementation of the treatment plan. Responsible for implementing performance outcome evaluations. With the direct input from the child and family, enlists the involvement of formal and informal community resources to support the family. Sees the family weekly for periods of time/days that may vary depending on the need of the family. Discharge planning/transitional service development. Responsible for all required record keeping as indicated by county and agency. Provides prompt intervention in the event of a crisis both to stop the crisis and to notify when indicated, persons and agencies necessary for the resolution of the crisis situation; provides "on-the-spot" counseling that is both helpful to the clients and consistent with the philosophy of the program. Attends to the safety, health, and well-being of clients. Responsible for periodic after hours on-call duties on a rotational basis. Attends child/family team meetings at other lead provider agencies as needed. Commitment to wrap around philosophy when working with families.
MINIMUM QUALIFICATIONS AND CERTIFICATION & LICENSES
Education, Training and Experience A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from an accredited college or university with a bachelor's degree in Social Work, Psychology, or a related field and four (4) years of varied experience counseling mentally disabled persons. Licenses; Certificates;
Special Requirements:
California driver's license & current vehicle insurance/registration if driving; and, Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles.
Schedule:
Monday-Friday, 9:30-6:00
Compensation:
