La Puente, CA Job Details Full-time $26 - $28 an hour 1 day ago Benefits Health insurance Dental insurance Paid time off Food provided Qualifications Bilingual Mandarin CPR Certification English Master's degree Case management Discharge planning Full Job Description About us La Puente Adult Day Health Care (ADHC) is a community-based program providing a variety of medical, rehabilitative, therapeutic, and recreational activities and services for older and/or disabled adults. We are looking for a qualified social worker to fill a few open positions at the center.
Our work environment includes:
Modern office setting Food provided Qualifications Bachelor's in Social Work or Master Degree in Social Work/Gerontology/Psychology/Counseling Day Shift (Required) Mandarin (Required) Work authorization (Required)
ADHC:
1 year (Preferred)
Social Work:
1 year (Preferred) Case management: 1 year (Preferred)
Leading Groups:
1 year (Preferred) Cantonese (Preferred) Job Responsibilities Complete monthly counseling, reassessment assessment, quarterly notes, discharge plans, meeting notes, home visits and biopyschosocial assessments. Interact with center participants on a daily basis, assist with inquires, such as SSI, Calfresh, senior housing, Medical and Medicare benefits. Intake Assessment (Center tour, documentation, eligibility check and follow up as needed.) Complete assessments, progress notes, flow sheets, and quarterly reports in a timely manner. Other duties as assigned Must be bilingual in Chinese (Mandarin and/or Cantonese)
