Job Description

Overview Join our compassionate healthcare team as a Hospice Social Worker, where you will play a vital role in providing emotional support, counseling, and case management for patients facing life-limiting illnesses. In this dynamic position, you will collaborate closely with interdisciplinary teams to deliver holistic, patient-centered care that enhances quality of life during challenging times. Your expertise in social work, mental health counseling, and hospice care will help families navigate complex emotional and practical needs with dignity and compassion. Duties Conduct comprehensive patient assessments to evaluate emotional, social, and psychological needs within hospice settings. Develop individualized care plans that incorporate behavioral health strategies, crisis intervention, and family counseling. Provide individual and group psychotherapy sessions utilizing techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy. Coordinate with medical teams to ensure seamless integration of social work services into overall patient care, including documentation in medical records and adherence to HIPAA regulations. Assist families with grief support, advanced care planning, and navigating healthcare systems, including assisted living or home & community care options. Collect data for clinical research initiatives related to hospice care outcomes and behavioral health interventions. Manage intake processes, facilitate referrals, and maintain accurate documentation using electronic medical record (EMR) systems. Requirements Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program; licensure or certification as required by state law. Proven experience in hospice care, inpatient settings, or home & community-based services. Strong knowledge of medical terminology, ICD coding (ICD-10/ICD-9), CPT coding, and medical documentation standards. Expertise in child & family counseling, mental health counseling, crisis management, and working with individuals with disabilities or developmental disabilities. Familiarity with behavioral health therapies such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and trauma-informed approaches like PTSD care. Ability to work effectively in fast-paced environments including inpatient facilities, nursing homes, or outpatient clinics; experience with EMR systems is preferred. Excellent communication skills for conducting diagnostic evaluations, case management, and facilitating group therapy sessions. $75.00 - $85.00 per hour
