Head of Service/LCSW/LMFT Definition:
Professionals in training to develop the skills necessary to support individuals, families and groups enhance their social functioning and overall well-being. Under the direct supervision and guidance of the LCSW/LMFT/Head of Service, associates will provide direct client care and help support success of their clients through positive collaborations with our Clinical Staff and other Providers in the community.
Duties:
1. Providing crisis intervention, intake assessment, and individual, family, and group therapy to adolescents with a history of mental illness. 2. Fill in as "Head of Service" when the Head of Service is not onsite. 3. Coordinating support services to assist residents with after care services. 4. Maintain a productivity standard of 70% of work schedule. (40 hours a week equals 28 billable hours per week, 173 hours a month equals 121 billable hours per month). 5. Provides crisis intervention/therapy and facilitates conflict resolution. 6. Plans and administers Individual Group, Family sessions as prescribed by the Integrated Core Practice Model. 7. Provides a range of services including individual, family therapy, and community based social rehab activity, collateral contacts and case management services. 8. Completes resident psychosocial assessments and provides diagnoses: formulates a plan of treatment which promotes development and maintenance of individual goals designed to facilitate individual growth, improved social interaction, ability to manage mental illness and live independently. 9. Assesses and monitors the condition of the resident to evaluate success of treatment and adapts interventions as needed. 10. Provides case management to include assessment of the resident/family needs, referral, links and support in accessing community services, psychiatry, medical care, housing, rehabilitation programs and social activity groups 11. Interfaces and collaborates with the psychiatrist regarding resident treatment planning. Updates the psychiatrist as to any changes in the residents' status (e.g. Incarceration, hospitalization, case closed). 12. Monitors and maintains documentation of the resident's services within the resident's charts, ensuring compliance with the Agencies policies and procedures, Department of Mental Health contract and other related agencies' standards and guidelines. 13. Develops and maintains therapist skills through seminars, workshops, or other forms of training/education related to working with adolescents with a history of mental illness. 14. Attends and participates in program, Agency staff meetings: and related community agency meetings as required/requested. 15. Arrives to weekly individual/group supervision prepared with cases to discuss and ready to receive feedback and direction from the supervisor. 16. The employee will act as a representative of Fleming and Barnes dba Dimondale Adolescent Care Facility in all of their interaction with DMH, LA County Probation, and
DCFS. 17.
The employee will provide aftercare services to residents who transition out of the
STRTP. 18.
Annually complete 40 hours of required content area training for CCL, CARF, DCFS, Probation, and DMH. 19. Must be employed for 40 hours a week and available for on call hours.
Requirements:
Must possess a Master's degree in Social Work or Marriage and Family Therapy, and be registered with their state licensing board as an associate throughout their volunteer time. Must maintain proper liability insurance throughout their volunteer time. Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Child Abuse Index Check (CAIC) background clearance. Must display effective communication skills, ability to adapt to various demands of clients, must show ability to demonstrate dependability, willingness to show initiative in carrying out responsibilities. Associates will be provided with weekly supervision by a licensed LCSW or LMFT in accordance with state requirements.
