Job Description

Overview We are seeking a dedicated and experienced Master Social Worker to join our dynamic healthcare team. In this vital role, you will provide expert social work services across various settings, including home health. Your leadership will help coordinate comprehensive patient-centered care, support families through complex challenges, and ensure compliance with all regulatory standards. This position offers an exciting opportunity to make a meaningful impact on individuals' lives while working within a collaborative and compassionate team. Duties Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, to develop tailored treatment plans. Manage crisis intervention situations with professionalism and sensitivity, ensuring safety and stabilization for patients experiencing acute distress. Collaborate with multidisciplinary teams to coordinate care plans that address medical, psychological, and social needs of patients with complex conditions such as dementia, PTSD, autism spectrum disorder, or developmental disabilities. Maintain accurate medical documentation in compliance with HIPAA regulations and hospital policies; utilize EMR (Electronic Medical Records) systems for data collection and record keeping. Qualifications Master's degree in Social Work (MSW) from an accredited institution; licensure or certification as a Licensed Clinical Social Worker (LCSW) or equivalent required. Proven experience in clinical counseling within hospital settings or hospice care environments preferred; experience working with children, geriatrics, or individuals with disabilities is highly valued. Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management. Familiarity with EMR systems and HIPAA compliance standards essential for maintaining confidentiality and accurate documentation. Excellent communication skills to engage patients across diverse populations including those with autism, dementia, or developmental disabilities. Ability to handle sensitive situations with empathy while managing multiple priorities effectively.
