Job Description
Company Overview Golden Castle Adult Day Health Center is a licensed community-based adult services program dedicated to providing compassionate care to older adults and individuals with chronic medical, cognitive, or mental health conditions. Our mission is to support personal independence through a coordinated program of professional services in a safe and nurturing environment. Summary We are seeking a dedicated Social Worker to join our team at Golden Castle ADHC. In this role, you will provide essential support and counseling to our participants, helping them navigate their social and health needs while ensuring they receive the highest quality of care in a community-based setting. Responsibilities Conduct assessments to determine the social and emotional needs of participants. Develop and implement individualized care plans in collaboration with participants, families, and healthcare providers. Provide counseling and support to participants dealing with mental health challenges. Facilitate group therapy sessions and activities that promote social engagement. Coordinate with community resources to enhance service delivery for participants. Monitor participant progress and adjust care plans as necessary to meet evolving needs. Requirements Master's degree in Social Work (MSW) from an accredited institution required. Experience in mental health counseling or geriatric care preferred. Strong communication skills and ability to work collaboratively with diverse populations. Knowledge of community resources available for older adults and individuals with disabilities.
