Olive Crest is dedicated to preventing child abuse by strengthening, equipping, and restoring children and families in crisis… One Life at a Time.® Overview Join our team to facilitate the Wraparound process for Child and Family Teams. Coordinate closely with family members, system partners, and stakeholders to develop Plans of Care and Crisis and Safety Plans within Wraparound timelines. Facilitator (Case Manager) - Wraparound Program Wraparound is a home based services program designed to help families at risk overcome obstacles and remain united. Facilitators must guide the Child and Family Teams in the development of Plans of Care and Crisis and Safety Plans according to Wraparound timelines.
Job Duties and Responsibilities:
Provide Wraparound services to children and families, including service coordination and implementation, facilitate Child and Family Team (CFT) meetings, linkage services, 24/7 crisis response. Cultivate relationships with county social workers, school personnel, mental health professionals, and others relevant to cases. Initiate and complete all documentation including strength summary, plans of care, crisis and safety plans, and progress notes. Promote Wraparound principles like family-centered practice, flexible service delivery, parent/family professional partnership, family decision making, collaboration across systems, community service delivery, inclusion of family and community resources, and strength-based practices.
Required Qualifications:
Bachelor's Degree with two years of relevant experience OR Master's Degree OR four years of relevant experience Proficiency with MS Office Strong organizational, time management, interpersonal, and communication skills Criminal background clearance Valid California driver's license and reliable transportation First Aid/CPR certification Ability to physically intervene in crisis situations with minors Alignment with Olive Crest's mission, vision, and values Philosophy supporting at-risk children and families Eligibility to work under mental health funded programs Bilingual in Spanish Health Benefits include: 2 Cigna Medical HMO's and 2 PPO's to choose from A Dental HMO and PPO through Cigna to choose from A vision plan through Cigna Time Off Benefits include: Up to 48 hours of sick time per year that caps at 120 hours Flexible use of sick hours for personal time Up to 1 week of vacation time for the first year, 2 for the second and an increased schedule every 5 years there after Up to 40 hours per year for Jury Duty or Trial Witness Up to 48 hours per year for Bereavement with death in immediate family Other available benefits for FT employees: 403(b) Retirement Plan Life Insurance Pre-Taxed Child Care HSA/FSA options 5 AFLAC Options to choose from Employee Assistance Program Holiday pay for shifts that fall on our 12 recognized holidays
