Full-Time / Part-Time / Per Diem Position Summary The Medical Social Worker (MSW) is a key member of the hospice interdisciplinary team responsible for providing psychosocial, emotional, and practical support to hospice patients and their families. The MSW assesses patient and family needs, develops care plans, facilitates access to community resources, provides counseling and crisis intervention, and assists patients and families in coping with end-of-life issues. The MSW promotes patient dignity, quality of life, and emotional well-being while supporting the mission and values of Reliant Hospice, Inc. Essential Duties and Responsibilities Conduct comprehensive psychosocial assessments of hospice patients and families. Develop, implement, and update individualized plans of care in collaboration with the interdisciplinary team. Provide emotional support, counseling, and crisis intervention to patients, families, and caregivers. Assist patients and families with advance care planning, healthcare directives, and end-of-life decision-making. Educate patients and families regarding available community resources, financial assistance programs, and support services. Facilitate referrals to appropriate community agencies and resources. Participate in interdisciplinary team meetings and contribute to patient care planning. Assist with discharge planning and coordination of services when appropriate. Provide bereavement support and counseling as assigned. Advocate for patient rights and promote patient-centered care. Maintain accurate, timely, and complete documentation in accordance with hospice policies and regulatory requirements. Participate in quality improvement activities and ongoing staff education. Maintain patient confidentiality and comply with HIPAA regulations. QualificationsEducation Master's Degree in Social Work (MSW) from a school accredited by the Council on Social Work Education (CSWE). Experience Minimum of one (1) year of social work experience in a healthcare setting preferred. Hospice, palliative care, home health, geriatric, or medical social work experience preferred. Licensure/Certification Current California Social Work license (LCSW) preferred, if applicable. Current driver's license, reliable transportation, and automobile insurance. CPR certification preferred. Knowledge, Skills, and Abilities Knowledge of hospice philosophy and end-of-life care principles. Strong assessment, counseling, and crisis intervention skills. Ability to communicate effectively with patients, families, physicians, and healthcare professionals. Knowledge of community resources and social service programs. Excellent organizational and documentation skills. Ability to work independently and as part of an interdisciplinary team. Compassionate, empathetic, and culturally sensitive approach to patient care. Physical Requirements Ability to travel to patient homes, facilities, and community locations. Ability to sit, stand, bend, and lift up to 25 pounds occasionally. Ability to perform job duties in a variety of home and facility environments. Working Conditions The Medical Social Worker provides services in patients' homes, skilled nursing facilities, assisted living facilities, residential care facilities, hospitals, and other community settings. Exposure to emotionally sensitive situations related to terminal illness and end-of-life care is expected.
