$23 to $25 an Hour The Case Manager for Minors and Non-Minor Dependents in the Transitional Housing Program (THP) provides direct support and intensive case management services to assist Transitional Aged Youth (TAY) ages 16 to 21 years, with meeting individualized goals while connecting them with resources in the community. The position is collaborative and coordinates care on an interdisciplinary team. As with all UC staff, this position is responsible for adhering to the performance standards set forth in the UC Staff Employee and UC policies and procedures.DIRECT REPORTS

:Required:
Bachelor's Degree in one of the following areas: Marriage, Family and Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Clinical Psychology, Educational Psychology, Education with an emphasis on Counseling, Social Work or Social Welfare, an area that includes counseling or psychotherapy content required for licensure as a Licensed Professional Clinical Counselor OR , a Bachelor's Degree in one of the following areas: Human Services or Behavioral Science, AND two (2) years of experience in a public or private child welfare social services setting, with specific experience working withTAY 16 - 24
years of age. OR, CCL approval as an Excepted Case Manager prior to start date Experience working with the TAY population Knowledge of Microsoft Offices applications Demonstrated skills in case work, group work and case coordination Understanding of transformational care planning and targeted case management principle of service delivery Excellent interpersonal skills (verbal and written) with demonstrated leadership in facilitating and linking services among a diverse variety of mental health programs Ability to maintain a positive perspective in the treatment and care of emotionally disturbed youth and young adultsPreferred:
Master's Degree in one of the following areas: Marriage, Family and Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Clinical Psychology, Educational Psychology, Education with an emphasis on Counseling, Social Work or Social Welfare, an area that includes counseling or psychotherapy content required for licensure as a Licensed Professional Clinical Counselor Two (2) years' experience working with at-risk youth or adolescent children in a residential group home setting Bi-Lingual in Spanish and EnglishRESPONSIBILITIES INCLUDE
(but not limited to):Program Implementation and Direct Client Services :
Utilize a variety of approaches to establish and maintain a relationship with youth Implement appropriate supervision, authority, and discipline to set limits for behavior and help each resident practice self-control Facilitate a high level of youth engagement Ensure a health home environment and client-focused programming Maintain individual contact with each youth at weekly: Discuss goals and track progress towards goals Understand any medications that they are prescribed, helping them learn how to refill prescriptions as needed Connect youth with community resources Ensure ongoing contact with outside persons and entities such as: natural supports, family, referring agency/case carrying agency, ILP, other and community resources Support ECM and Mental Health services based on trainings by managed care plans and county contracts.Support youth:
With the development of independent living skills Support the goals outlined in the Transition Independent Living Plan (TILP) and/or the Appraisal Needs and Service Plan Work with youth to identify additional goals and tasks related to employment, education, and housing Establish and monitor client budgets and submit purchase requests Aid youth in their transportation goals, providing guidance on public transportation options and navigating the DMV All social work activities with clients is supervised by the social worker supervisor, generally the Program Manager or Program Director Create care plans and ECM screenings such as depression screening and blood pressure screening. Participate in relevant team meetings such as; Child Family Team meetings, TDMs, and emancipation conferences THP Program Statement Maintain knowledge of THP Program Statement Implement services per the THP Program Statement Attend required training to maintain knowledge and certification needed for the performance of the role. Staff will be required to cover shifts as needed and on an emergency basis.Compliance and Reporting :
Responsible for completing reports, notes and Medi-Cal paperwork in a timely manner, as applicable Responsible for inputting data into the client database within 2 days of change or receipt of information Ensure records, documents and paperwork is up-to-date and maintained on-siteCCL Regulations:
Maintain knowledge of CCL Regulations Ensure continuous compliance with CCL and all regulations and external requirements Conducts regular audits and monitors homes where clients reside to ensure compliance with CCL regulations.Program Productivity :
Case Managers are expected to meet productivity standards, as applicable. Maintain accurate and billable case notes per Medi-Cal requirements, as applicable. Ensure 75% of youth graduations Ensure all clients are evaluated and assessed with proper documentation and established assessmentsProgram Ambassador :
Serve as community liaison and agency representative Interface with County Child Welfare and Probation staff on all facets of case coordination Research and develop community resources for youthOrganization Duties :
Timely and accurate submission of timecards to supervisor Time away from the Office, Program and/or Home (i.e. vacations, personal time off, leaves of absence) must be pre-approved by supervisor and requested based on process outlined in the UC Employee Handbook. Maintain knowledge of staff handbook, policies, procedures, manuals, and pertinent documentation Other duties as assigned.ESSENTIAL

Employee must occasionally lift and/or move up to 25 pounds. Employee may be required to make or receive physical contact with clients and residents, such as in the process of delivering First Aid/CPR or de-escalation interventions. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Must have CPR and First Aid certificate within 10 days of start date and maintain a current certificate with Human Resources. Must clear a Department of Justice background check including fingerprinting before start date and maintain clearance. Must be able to work in a DSS Community Care Licensed Facility. Must clear a health screening, including TB before start date and recertify TB annually. Must have a car, valid California Driver License and be able to drive, have a clean driving record and maintain a clean driving record. Meet and maintain the minimum vehicle liability and property insurance limits (to be insured by our insurance carrier for using vehicle for business purposes such as transportation of residents or reimbursement of business-related mileage expenses).Disclaimer :
