case management services through the AIDS Medi-Cal Waiver Program . This position serves a diverse client population in their home setting and focuses on helping clients remain stable, connected to care, and supported in the community. Responsibilities Partner with an RN to provide supportive, client-centered
HIV/AIDS
case management services in the home setting Conduct biannual assessments and maintain monthly follow-up with clients between home visits Assist clients with accessing and remaining connected to appropriate medical, mental health, AOD, and community-based services Help stabilize clients' health and social needs while working to avoid unnecessary higher levels of care Maintain accurate and timely documentation of client interactions, assessments, and care coordination activities Build trusting relationships with clients while providing compassionate and culturally sensitive support Collaborate with community providers and internal team members to ensure continuity of care Education/Training Minimum of a Bachelor's Degree in Social Work, Counseling, Psychology , or a related field Equivalent degree qualifications may be considered Experience Minimum of 2 years of experience providing case management or related services Experience working with
HIV/AIDS
