Social Work Supervisor, Director of Nursing About the Role We are seeking a compassionate and skilled Renal Social Worker (ACSW) to join our multidisciplinary nephrology care team. In this role, you will deliver critical psychosocial services to patients living with chronic kidney disease (CKD), end-stage renal disease (ESRD), and those undergoing dialysis or transplant preparation. You will support patients and families through emotional adjustment, care coordination, resource navigation, and education to enhance quality of life and treatment outcomes. Key Responsibilities Conduct comprehensive psychosocial assessments for renal patients and their families. Develop, implement, and monitor individualized care plans aligned with treatment goals. Provide counseling and emotional support to help patients and caregivers cope with chronic illness, dialysis, and transplant decisions. Advocate for patients' needs within the healthcare system and with external agencies. Assist patients with navigating insurance issues, financial concerns, community resources, and support programs. Educate patients and families about kidney disease, treatment options, and lifestyle adjustments. Collaborate with physicians, nurses, dietitians, and other care team members in interdisciplinary meetings . Maintain accurate patient documentation in medical records. Participate in quality improvement initiatives and compliance with regulations. Qualifications Active clinical licensure (ACSW, LCSW, or equivalent) in the state of practice. Previous experience in healthcare or medical social work; nephrology or renal experience preferred , but not always required. Strong knowledge of psychosocial assessment, care planning, crisis intervention, and community resources. Excellent communication, advocacy, and interpersonal skills. Ability to work effectively within a multidisciplinary team. Spanish Speaking highly preferred •
