WORKING HOURS
: The Tribal WrapAround Lead Care Coordinator is an exempt position that provides care coordination case management services to adults and/or youth who need housing resources often due to unmet social, health, behavioral health needs and may have criminogenic needs. This lead position provides supervisory support to other care coordinators and peer supports in the program. The Lead Care Coordinator reports to Wraparound Program Manager and assists with supervisory duties and day to day operations. This position has regular business hours but often requires work in early mornings, evenings, and weekends. May be required to perform after-hours standby services as needed or some evening work, especially during the winter shelter season.
SUMMARY :
The Tribal Wraparound Lead Care Coordinator provides supervisory support and managers a small care coordination case load of clients. The Lead supports other care coordinators in day-to-day operations of navigating barriers that arise in services, connecting to system partners, ensuring client engagement, and reviewing plan of cares. The Lead supports the program manager in ensuring that care coordination activities meet grant requirements, data is being tracked appropriately and communicating progress towards grant deliverables. Care coordinators are responsible for providing intensive care coordination for Tribal individuals, families, and youth who have been identified as being high risk for homelessness or is currently homeless. Care coordinators identify system partners such as Indian Health Services, Behavioral Health, Probation / Parole, Courts, Elder & Disability Services, Veteran Services, and Child Welfare to bring them together as tribal wraparound team to help empower the client toward self-sufficiency and independence. The position fills a central role in linking homeless individuals and families to an appropriate housing plan of care and supporting them in tenancy sustaining services. The goal is to facilitate rapid and resourced exits from homelessness into permanent supportive housing and provides participant advocacy, case management, benefit establishment, linkage to Mental Health and/or Substance Use Services, linkage to stable housing and all other supportive services as needed. The lead assists with supervision of other care coordinators intensive case management services. This position is funded in full or in-part through Homeless Housing, Assistance and Prevention (HHAP), Housing and Disability Advocacy Program (HDAP), Bringing Families Home (BFH), Behavioral Health Bridge Housing (BHBH), HomeSafe, Native American Housing Assistance and Self Determination Act (NAHSDA). The worker will be assigned a focus population but cross-trained in all areas and expected to assist with other focus populations based on company and community need.
ESSENTIAL DUTIES AND RESPONSIBILITIES
: 1. Maintains a caseload of 7 with at least one hour of face-to-face contact per week per client while promoting tribal wraparound philosophy and practices. 2. Under direction of the Program Manager, supervises Tribal Wraparound Care Coordinators and Peer Supports, ensuring quality services are delivered in timely manner. Maintains availability for providing day to day case consultation and guidance as needs arise on the team. Maintains clear communication with Program Manager and escalates complex needs appropriately. 3. Reports client issues or complaints to Program Manager immediately or as soon as possible. 4. May be asked to join Program Manager in weekly case consultations with staff. 5. Attends every Tribal Wraparound weekly staff meeting and co-facilitates meeting. 6. Consults with Clinicians when mental health issues or crisis needs arise. 7. Assists with development, updating, and implementation of Tribal Wraparound Program policies and procedures. 8. Ensures that care coordination activities meet grant requirements and communicates progress towards grant deliverables. 9. Supports care coordinators in proper documentation, quality control, and data collection. Assists program manager in quarterly review of client chart audits. 10. Lead Care Coordinator is first reviewer of plan of cares and can approve plan of cares without complex needs. Consults and elevates complex needs to program manager for final approval. 11. Assists program manager in tracking training completion and needs. 12. Assembles a Tribal WrapAround Team within two (2) weeks of referral by interviewing the client, identifying family members/natural supports/agency representatives and other significant persons. Uncovers the client's strengths and needs, provides assistance with any immediate needs, as well as completes the initial Crisis Plan. 13. Within the first 30 days, coordinates Team Meetings and develops the initial individualized Plan of Care (POC) based on the identified strengths and needs, including a comprehensive Crisis Plan. The Plan should reflect the best possible fit with the culture, values and beliefs of the client. 14. Monitors the provision and quality of services provided to the client and family through the Team and is the liaison when new services/resources need to be sought or developed. 15. Provides or arranges for transportation for his/her clients to appointments, crisis/emergency services, etc., if needed. 16. Provides or secures support and crisis/emergency services for the client. 17. Completes all the necessary paperwork in a strength-based manner and maintains accurate information on-line, ensuring that demographic information is up-to-date, as well. 18. Promotes collaborative relationships with other system partners such as Indian Health Services, Behavioral Health, Probation / Parole, Courts, Elder & Disability Services, Veteran Services, and Child Welfare to bring them together as tribal wraparound team to help empower the client toward self-sufficiency and independence. 19. Shall dress in a respectful manner and always maintains and promotes professionalism among staff members. 20. Provides written and verbal information related to the client's behavioral and mental health based on assessment and contact. 21. Provides a comprehensive community-based safety plan with innovative strategies to assist the client in being maintained safely in the least restrictive setting. 22. Collaborates and partners with inter and intra agencies. 23. Submits reports on a timely basis. 24. Assists in the development and expansion of the Housing Department Programs as directed. 25. Meets with leadership, clinical, and consultation for discussions and clarification on various issues as necessary. 26. Attends all required meetings and training. 27. Performs other duties as assigned.
SUPERVISORY RESPONSIBILITIES
: Responsible for supervising wraparound team of care coordinators and peer supports. Position is overseen by wraparound program manager.
QUALIFICATIONS
: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and abilities required. 1. Bachelor's Degree in Social Work, Sociology, Criminal Justice, Human Services or related field is required. Masters degree is preferred. 2. Two or more years of relevant experience and a demonstrated track record in a similar related area. At least 6 months of supervisory or increased responsibility in training/supporting other staff. 3. Knowledge of grant program implementation and reporting. 4. Honesty, accountability and transparency with a commitment to social justice. 5.

Excellent crisis de-escalation and communication skills are required. 7. Demonstrated intellectual independence and initiative and willingness to learn new skills. 8. Must be able to maintain a high degree of confidentiality concerning all matters about the Tribe. 9. Extensive knowledge and experience working with Native American communities, culture, customs and cultural law.
TRIBAL AND INDIAN PREFERENCE

EDUCATION
