Full-time Job Summary:
The Grief Support Counselor and Group Coordinator is a permanent member of the GSP team and holds a current LSCW or LMFT licensure in the state of California. The GSCC carries a regular caseload of 1:1 clients while coordinating referrals, screening and organizing logistical details of HSCC grief support groups. The GSGC communicates and coordinates with clients, staff, volunteers, and members of the community as needs arise.
Essential Responsibilities:
Carry a caseload of 10-15 clients per week, while offering 3-4 intake times per week (45-minute call) Facilitates the Drop-in Group weekly. Coordinates having a co-facilitator (Volunteer) for each group. Coordinates ongoing support groups and places volunteer co-facilitators in groups. Provides training and ongoing support and feedback to volunteers. Provides clinical oversight for groups and 1:1 counseling Assists in design, implementation of grief group interventions Is responsible for coordinating consents, intakes, and group location details of in-person and online groups in collaboration with GSP Admin Conducts intake assessments as needed and initiates plans of care Revises, updates, and closes care plans as needed Collaborates with Marketing team to promote services through outreach Maintains high level of communication with clients, volunteers, staff, and community at large regarding groups Consultation as necessary regarding group and individual clients
Specific Functional Responsibilities:
Participate in volunteer and staff trainings as requested Participate in GSP events as appropriate Serves as resource/liaison to Patient Care Department as needed Performs other related job duties as assigned by GSP manager
Compliance:
Ensures compliance with all job-related State/Federal Laws; ensures all HIPAA Privacy and Security Regulations, Medicare Conditions of Participation, and all other regulatory/compliance requirements are understood and followed.
Supervisory Responsibility:
Oversees volunteers who co-facilitate groups and who perform 1:1 counseling. Requirements Education/Experience Licensed MFT or
LCSW 1-3
years of group coordination and facilitation Grief Support experience highly desired Skills Required Pass OIG/GSA review and all required background checks Strong, clear communication skills
Certifications, Licenses, Registrations:
Current California LCSW or LMFT licensure Physical Demands Strength to lift and carry materials weighing up to 10 pounds Vision to read printed materials and a computer screen Hearing and speech to communicate in person and over the telephone Work Environment Must possess mobility to work in a standard office setting and to use standard office equipment, including a computer
